Existing Grantees

The Livewell Foundation - reporting guidelines

The Livewell Foundation expects successful grantees to monitor the delivery and success of their project throughout the grant period to ensure that the aims and objectives of the grant and the project are being met. 

Please submit a (1) midway and (2) end of grant report to The Livewell Foundation on thelivewellfoundation@gmail.com 

Please include the following details:

1. Name of organisation

2. Name of person completing report

3. Your email address

4. Project start date

5. Date of this report

6. Project title

7. Please highlight whether the report is (a) midway or (b) final

8. Total amount of grant received

9. Total amount spent on project to date

10. Please outline any reason for underspend at this point

11. Written reports - please detail any activities, outputs and / or outcomes your project achieved.  Please attach any testimonials, quotes or case studies that depict the impact your grant has achieved so far.  There is no word count for this – please include as little or as much information as you feel necessary for your update.

12. We now also accept video reports. Videos should be no longer than 5 minutes long. We suggest you save your video on a You Tube or Vimeo platform and send us the link. Again, please detail any activities, outputs and / or outcomes your project achieved.  Please attach any testimonials, quotes or case studies that depict the impact your grant has achieved so far.
